Everyone employed at CHOICES signs a confidentiality agreement. This agreement stays in force even if the person leaves the employ of CHOICES.
We collect personal information to help us provide supports to our clients. We also collect contact information to keep you up to date on what is happening here at CHOICES and to ask you to give us feedback on how we are doing. We also collect contact information from individuals and businesses who donate services, money or other forms of support to the agency.
Information we collect may include; email addresses, home/business addresses and other contact information, your marital status and your decision making relationship with our client. i.e. are you a guardian or trustee.
For those who have a direct support relationship with out clients and with consent, we share at least your contact information with third parties such as government agencies and other service providers involved in funding or providing the support services for the client. We may also share it with the medical community if it becomes necessary to do so. These other agencies are part of the circle of support for our clients.
We do not sell your personal information or share it with any other third parties not involved in funding or providing support to our clients.
