Supported Group Living Residences (SGLR)

With the support of Developmental Services Ontario (DSO), the matching process at Choices is designed to ensure that each person is supported to live in an environment where they are compatible with their peers and where the setting aligns with their individual needs.

 

Across all Supported Group Living Homes, Choices provides a consistent approach and a level of support that exceeds expectations.

Through the Person-Centred Planning Model, people supported are encouraged to identify and pursue their own goals — fostering personal growth, independence, and community participation.

 

In collaboration with a wide range of community partners, Choices is committed to creating opportunities that reflect and celebrate the uniqueness of every individual.

Supported Independent Living (SIL)

The Supported Independent Living (SIL) program provides individualized supports to adults with developmental disabilities, empowering them to live independently within their community. The program promotes personal growth, skill development, and community inclusion through a person-centred and trauma-informed approach.

 

SIL supports individuals in a variety of living arrangements — including those living on their own, with family members, or with roommates.

Supports are tailored to each person’s needs and may include assistance with budgeting and banking, maintaining housing, building community connections, and navigating services. The level of support varies from a few hours each day to a few hours each week, depending on individual goals and requirements.

 

Host Family

This program connects adults with developmental disabilities to families in the community, offering the opportunity to live in a supportive family environment that aligns with their personality and care needs. Choices is responsible for screening, approving, and providing ongoing supervision of these living arrangements to ensure safety, compatibility, and quality of care.
